Yes, the Exclaimer Outlook Add-in and the Exclaimer Signature Update Agent utilize Microsoft authentication mechanisms that fully support MFA.
No, enabling MFA on the Global Admin account with which Exclaimer was configured will not cause any problems. The Global Admin account is only necessary to grant permissions for the Azure AD apps that Exclaimer uses. The account is not retained as a service account.
Exclaimer uses MFA to authenticate sign-ins on a per-browser basis. The initial login will require MFA, but subsequent logins from the same browser will not require MFA authentication unless the browser data is cleared.
Exclaimer Portal users can choose to configure Microsoft or Google SSO for their account; however, it is not currently possible to enforce that these SSO options are used.
